Welcome to Back 2 Normal Physical Therapy! By accessing or using our website, services, and programs, you agree to comply with and be bound by the following Terms and Conditions. Please read them carefully.

1. Acceptance of Terms

By using our services or website, you acknowledge that you have read, understood, and agree to be bound by these Terms and Conditions. If you do not agree, you must not access or use our services.

2. Services Provided

Back 2 Normal Physical Therapy provides:
✔ Physical therapy, wellness, and performance enhancement services tailored to individual needs.
✔ Virtual and in-person consultations, educational materials, and recovery tools.
✔ Online appointment scheduling through Mindbody software.

All services are subject to availability and may vary based on the specific plan agreed upon with the client.

3. Client Responsibilities

Clients are responsible for:
✔ Providing accurate and complete medical history and relevant health information.
✔ Following prescribed treatment plans and attending scheduled appointments.
✔ Informing us of any changes in medical conditions or symptoms.
✔ Ensuring payments are made in a timely manner.

4. Appointment Scheduling and Cancellation

Scheduling: Appointments can be scheduled by calling (727) 362-6866, through our contact page. Note: Some select appointments can be requested via Mindbody software.

Cancellation Policy: Cancellations or rescheduling must be made at least 24 hours before the scheduled appointment by calling or texting (727) 362-6866. Email cancellations are NOT accepted. Failure to provide 24-hour notices or not go through outlined channels may result in a cancellation fee for the full price of the session.

No-Show Policy: Missed appointments without prior notice will be subject to fees for the full price of the missed session.

5. Payment Terms

✔ Payment is due at the time of service unless prior arrangements have been made.
✔ We accept cash, credit cards, and other approved payment methods.
✔ Prices for services and packages are available upon request and are customized based on individual client needs.

6. Refund Policy

Satisfaction Guarantee for Whole Body Evaluation

If you are not 100% satisfied with your Whole-Body Evaluation and do not feel confident that we can help you address your concerns, your evaluation fee will be waived. To qualify, you must notify us at the conclusion of your evaluation.

Service Refunds

✔ Refunds for service packages are assessed on a case-by-case basis.
✔ If a refund is granted, deductions may apply for completed sessions.
✔ The satisfaction guarantee only applies to the evaluation fee and does not extend to any additional services or treatments.

Retail Returns & Refunds

Thank you for your purchase. If you are not entirely satisfied with your purchase, we’re here to help.

The following refunds and returns policy apply only to non-affiliate retail items sold by Back 2 Normal Physical Therapy. Please see the Affiliate Policy below for retail specifics on ASEA, HeartMat, dōTERRA, Metagenics, Bemer, and VoxxLife.

✔ Return Window: You have 14 calendar days to return an item(s) from the date you purchased it.
✔ Condition Requirements:

• The item(s) must be unused and in the same condition that you received it.
• The item(s) must be in the original, untampered packaging.
  ✔ Proof of Purchase: You must present the receipt or proof of purchase.

Once we receive your item(s), we will inspect it and notify you that we have received your return. We will immediately notify you on the status of your refund after inspection.

✔ Refund Process: If your return is approved:

• We will initiate a refund via your original method of payment.
• If your purchase was made by check, we will place a credit on your account for future use.
• The refund timeline will vary based on your bank or card issuer’s policies.

Shipping for Returns

✔ You are responsible for return shipping costs.
✔ Shipping costs are non-refundable.
✔ If you receive a refund, the cost of return shipping will be deducted from your refund.

Affiliate Policy

For ASEA, dōTERRA, Metagenics, Apex Energetics, Bemer, HeartMath Archies and VoxxLife retail products, for which Back 2 Normal Physical Therapy acts as an affiliate in extending membership opportunities:

✔ Any items purchased through your personal membership or as part of a cooperative purchase between Back 2 Normal Physical Therapy and you (or your entity) are subject to the affiliate company’s returns and refunds policy.
✔ Back 2 Normal Physical Therapy is not responsible for returns, refunds, or affiliated costs for these products.

Policy on Services

✔ This refunds and returns policy does not apply to physical therapy services rendered by Back 2 Normal Physical Therapy.
✔ Please refer to your Client Services Agreement for service-specific policies.
✔ At any time, you can request a copy of the Client Services Agreement signed upon initiation of services.

7. Medical Disclaimer

✔ Back 2 Normal Physical Therapy is not a substitute for professional medical advice, diagnosis, or treatment.
✔ Clients should consult with their primary healthcare provider before beginning any new therapy or treatment plan.

8. Limitation of Liability

To the extent permitted by law:
✔ Back 2 Normal Physical Therapy shall not be held liable for any damages resulting from the use of our services or website.
✔ We are not responsible for injuries or adverse outcomes arising from non-compliance with prescribed treatments.

9. Privacy Policy & Data Protection

✔ Your information will not be sold, rented, or shared without your consent, except as required by law.
✔ Our Mindbody appointment scheduling software may collect your name, contact details, and payment information. Their data policies apply and can be reviewed here: Mindbody Privacy Policy.

10. Intellectual Property

✔ All content on the Back 2 Normal website, including text, graphics, logos, and images, is the property of Back 2 Normal Physical Therapy.
