Our team provides a restorative environment with treatments and services that honors a philosophy of treating the whole person. We take into account the subtle balance that exists within your body and develop a treatment plan, which collaborates with fitness specialists and health/medical providers to help get you “Back 2 Normal”.
If you do not have a physician to refer you to physical therapy, we are happy to assist you in sourcing one to meet your needs.
Some insurance companies will not reimburse physical therapy services without a referral.
We recommend clients contact their insurance company to find out what their out-of-network benefits include, so they know what to expect regarding reimbursement.
Here are some questions for them to ask:
o Do I have out-of-network benefits?
o Do my out-of-network benefits include physical therapy?
o What is my out-of-network deductible?
o Once my deductible is met, what percentage is reimbursed?
o What is my Out-Of-Pocket amount for my out-of-network benefits?
o What is the effective date of my policy? DO my claims run on a calendar year or do they run based on effective date?
o Are there any forms I need to complete online and fax in with my invoice?
o Clarify the mailing address that the claim needs to be sent to. ( FYI on the back of a client’s insurance card there should be the mailing address for claims but it does not hurt while they already have them on the phone to confirm that’s an accurate address)
Any other requirements needed for reimbursement?
Clients may need to complete forms from their insurance company ( see above questions to ask insurance company) and mail in with the above invoice and insurance cover letter.
The client is responsible for payment at time of service. Back 2 Normal PT accepts credit cards, cash and checks for payment.
As a courtesy, Back 2 Normal Physical Therapy will provide each client with an invoice for all services rendered. The client will be responsible for paying at time of service and can submit for reimbursement to their insurance company.
Please come prepared with all necessary paperwork completed. Paperwork can be downloaded from our forms page. If you have any reports from your physician, or test results, please bring a copy for the therapist to review.
